It's been a while, but the San Francisco 49ers are heading back to the Super Bowl for the first time since in 18 years.

Because of their five titles, however, few NFL franchises are as linked so intimately to the biggest game in the sport. Sunday's thrilling comeback and late defensive stand for a 28-24 road win over the Atlanta Falcons sends the 49ers back to the title match for a sixth time.
